About me (the person making this report)

Disability Commissioner Tasmania protects the privacy of everyone who makes a report.

We can only share your identity if:

  • You have consented,

  • It is necessary to prevent significant harm to a person with disability, a child, or other vulnerable person, or

  • It is necessary for law enforcement.

For more information, you can read about our privacy and consent policy, or contact us to talk more.

Disability Commissioner Tasmania must contact everyone who makes a report to us.

If you do not want us to contact you (or someone else on your behalf), we will treat this as a tip-off rather than a formal report under the Disability Rights, Inclusion and Safeguarding Act.

We may still be able to investigate your tip-off if you can provide enough other details.

Please contact us to talk to someone at Disability Commissioner Tasmania about protecting your privacy.

You have the right to stay anonymous.

We may still be able to investigate your report if you can provide enough other details.

If you think you might want to tell us more about your report in the future, consider using a fake name (a pseudonym).

My contact details

You have asked us to come visit you in person, but you have not provided a phone number or email address.

Without a phone number or email address, we cannot contact you to tell you:

  • Whether we are coming to visit you

  • What time or day we are coming to visit you

  • Who from our team will be visiting you.

Our visit to you would be unannounced (a surprise).

Are you sure you want an unannounced visit from Disability Commissioner Tasmania?

Is there anything else we need to know about contacting you?

For example: "I need an Auslan interpreter", "text before calling", particular days or times that are safe to call, "Do not say who you are if [person] answers the phone", or something else.

My basic information

If you have more than one first name, enter the one you want us to use for you.

We will always respect your pronouns. You can change the pronouns you want us to use for you at any time.
It's okay if you need us to use different pronouns around different people.

Why we ask about your home address:

  • It helps us understand what's happening in different parts of Tasmania

  • It helps us identify you, in case someone else in Tasmania has same name as you

If you live at more than one address, use the address where you spend more time.
If you don't want to tell us your full home address, it's okay to just enter your postcode.

All of the questions in this section are optional. You do not have to answer them if you don't want to.

About the person (or people) with disability

The questions we ask in this section help us to:

  • Learn more about the types of issues that affect people with disability in Tasmania

  • Make decisions to protect the privacy of people involved with this report, and

  • Decide how we will make our  preliminary enquiries.

Preliminary enquiries are how we gather information about a report, so we can assess risk and make a decision about whether we will investigate.

Disability Commissioner Tasmania will contact the person with disability directly to talk to them about this report, as long as it is safe and practical to do so.

We will not share your identity with the person with disability, unless:

  • You have given consent

  • It is necessary to prevent significant harm to a person with disability, a child, or other vulnerable person, or

  • It is necessary for law enforcement.

The person (or people) with disability do not need to be aware that you are making this report.
This question helps us make decisions about how to approach our preliminary enquiries.

You do not need the consent of the person (or people) with disability to make a report.
This question helps us make decisions about how to approach our preliminary inquiries.

What is your relationship to the person (or people) with disability?
(For example, "I am his sister", "I am Fred's support worker", "I don't know them")

Please tell us as much as you can about the people with disability involved.
(For example: names, contact details, disabilities or impairments, any other details that may help to identify or contact them)

Please tell us as much as you can about the person with disability.

If the person needs to stay anonymous, or you don't know anything about them, leave this section blank.

(For example, "wheelchair user", "intellectual disability", "speech impairment", "had a support worker with them")

How can we contact the person?

My report

Please tell us about the situation you want to report.

This section is divided into:

  • Who

  • When

  • Where, and

  • What.

Who was involved? (Or who will be involved?)
(For example, the names of people or organisations, if you know them)

When did it happen? (please be specific, if you can)
(For example, "Tuesday 10th of June at about 3pm", "During January-February 2025", "I don't know")

When are you worried it might happen in the future? (your best guess is okay)

Please tell us more about  when you are worried it will happen:
(For example, "August 15th", "At my next therapy appointment", "When my father next visits", something else)

Where did it happen? (Or where are you worried it will happen?)
(For example, an address, a service location, "at the bus stop in Huonville", "at my mum's house", something else)

What happened?

What are you worried will happen in the future?

The next 4 questions are about other people or authorities who may be involved in the situation.

The answers to these questions may change the way we respond to your report.
For example, if police are involved, we need to make sure our preliminary enquiries do not interfere with any criminal investigations.

Have you made a report or complaint about this situation to anyone else?
(For example, "my service provider", "the Anti-Discrimination Commissioner", "the NDIS Commission", someone else)
