This form is for anyone to report a concern that relates to a Department of Premier and Cabinet (DPAC) worker. This includes specific incidents or general worries, for both current and historical concerns.

A 'DPAC worker' includes any individual over the age of 18 years who carries out work in any capacity (paid or unpaid) for DPAC. This includes an employee, a contractor or sub-contractor, an agent, an apprentice, a consultant, a person who is self-employed, a person who is carrying out work for a defined period, a volunteer, or a work-experience student.
